1 REAL ID required documentsAll documents must be originals or certified. Photocopies will not be of Citizenship or Legal birth certificate from State Office of VitalStatistics or equivalent with a raised and/or color sealValid, unexpired passport or passport certificate or consular report of birth abroad Valid, unexpired Permanent Resident Card issued byDHS or INS (I-551)(FS-240, DS-1350 or FS-545)These documents must show the applicant's legal name and full date of birth. Applicant mustpresent one (1) of the following documents:Unexpired Employment Authorization Documentissued by DHS (I-766 or I-688B)Unexpired foreign passport with valid visa andapproved I-94 formCertificate of Naturalization issued by DHS (N-550, Certificate of Citizenship issued by DHS (N-560 or N-570, or N-578)N-561)Marriage certificateIf there has been a name change from the Proof of Citizenship or Legal Presence presented,certified legal document supporting name change must be presented.

Applicant must presenttwo (2) of the following documents with one document dated within the last four (4) months:Receipt for personal property or real estate taxes paidwithin the previous yearInstallment loans, such as automobile, student loan,bank loansCurrent employer verification of resident address orletter from employer as long as it is on companyletterhead with original signature.
