Signature: _____ License #:_____ Date: _____ Phone #:_____ Please upload the completed document to the permit record in the HillsGovHub portal. 2 of 2 02/2021 Re- roof Inspection Information roof Mitigation If more than 25% of the total roof area or roof section of any building or structure is replaced or recovered in any 12 month period, the entire roof system or section must conform to current codes. Inspectors will check that roof attachment/s are strengthened and corrected as required per Hurricane Mitigation Retrofit Manual. Roofing Contractor can request a Progress Check inspection or can schedule a virtual inspection and send in supporting Pictures/Videos.

3 If no Progress Check inspection has been made by a Hillsborough County Building Inspector, the contractor must have a roof Mitigation Verification Statement filled out and uploaded through HillsGovHub before final inspection is requested. The purpose of this Verification Statement is to certify portions of the repairs, upgrades, and dry-ins which can no longer be viewed by the inspector. Homeowners installing their own roofing must either schedule a Progress Check inspection by the County inspector or have inspections performed and certified with the roof Mitigation Verification Statement by a Licensed Roofing Contractor, Licensed General Building Residential Contractor, a Licensed FS486 Building Inspector and/or a Registered Architect or Engineer in the State of Florida. This form will then need to be uploaded through HillsGovHub to the documents under the permit record. Final After the Progress Check inspections are performed or the roof Mitigation Verification Statement has been uploaded through HillsGovHub, a final roof inspection can be performed.
