We are taking this action because you have been non-compliant. You have failed to return our phone calls, failed to get lab work done, and missed your last three appointments. This makes it very hard to provide your ongoing care, and it is unfair to other patients who are waiting to be seen. We will see you for emergency care if needed, but only until September 1, 2016. This will give you time to find another physician to provide your ongoing care. We will send a copy of your records to your new provider.
