This Privacy Statement applies when you use this website to create a personal profile in relation to your NSW Health uniform allocation. For the purposes of this Privacy Statement, the NSW Public Health System includes all NSW Local Health Districts, Speciality Networks, Statutory Health Corporations and Affiliated Health Organisations. The personal information you provide via this website is collected by an external service provider, ADA, contracted by HealthShare NSW to coordinate the ordering and allocation process for staff uniforms. Much of this information, as recorded in the Human Resources Information System (HRIS), as provided by you at the commencement of your employment in the NSW Public Health System . What personal Information is collected through this website? As an employee in the NSW Public Health System who is required, or has the option to, wear a uniform, you will be asked to create a personal profile and review the personal information retrieved from the HRIS; including your name, sex, employee number, employment type/FTE status, and your position or classification.

3 What are the purposes of collecting this personal information and how is it used? In this instance, your personal information will be used for the following purposes: Allocation of garments appropriate to your position Distribution of garments to your nominated location Billing your department or service for allocated garments It may also be necessary for staff involved in any of the above processes to contact you should there be questions regarding your order. Who has access to your personal information? The information you supply though this website is utilised by ADA for the purposes outlined above.

4 Your Manager will also be able to view this information to verify allocations for billing purposes. HealthShare NSW staff may also view your information in the administration of the Total Apparel Management System (TAMS). Additional parties, such as law enforcement agencies and others who have a right at law to access your personal information (for example, through search warrants or court orders), may also apply to access your information. Storing and accessing your personal information Reasonable steps will be taken to destroy your personal information in a secure manner or remove identifying features from it when it is no longer required.
