This Program provides mobile medical care services at service-area homeless services agencies and provides linkage and referral back to SJWCFC s independent Health centers for follow-up care. The Program also includes homeless services case management (including linkage to housing assistance) and Community outreach to hard-to-reach populations to increase access to medical, dental, behavioral Health care and support services for homeless and housing insecure patients.
