This Program provides mobile medical care services at service-area homeless services agencies and provides linkage and referral back to SJWCFC s independent Health centers for follow-up care. The Program also includes homeless services case management (including linkage to housing assistance) and Community outreach to hard-to-reach populations to increase access to medical, dental, behavioral Health care and support services for homeless and housing insecure patients. Must be able and willing to work a minimum of 20 hours a week at a verifiable employment source. 2 letters of recommendation from a non-family adult and must be from different sources Provide a $100 deposit, and $50 will be awarded upon graduation Can be former DCFS but not currently in DCFS Can be former probation, except violent crimes, sex offenses, and arson Can have a history of mental illness, but not a major history or recently hospitalized (6 months) Tailored for those coming out of shelter or TLP youth Substance abuse is ok, but will be addressed in case plan Los Angeles Youth Network Location: 1550 Gower St.
