REV 4/6/2022 Page 1 of 4 Property Owner CLAIM Filing Instructions AND REQUESTED DOCUMENTATION To claim Property reported in your name, please provide the following: Claim Affirmation Form signed by the Property In general, a Power of Attorneyinstead of the reported Owner s signature will not be must notarize the Claim Affirmation Form if:1)The total value of the claim is $1,000 or greater;2)The claim is for safe deposit box contents; or3)The claim is for securities, such as:a)Stocks;b)Mutual funds;c)Bonds; ord) the Property lists multiple owners, please provide the following:1)Each Owner must sign a Claim Affirmation Form. Multiple owners may submit their ClaimAffirmation Forms together if they are sharing the same documentation to support )If the owners submit separate Claim Affirmation Forms, each claim shouldcontain all requested )If the Property is a safe deposit box, and all owners are living, a Safe Deposit Box PropertyRelease Form may be requested to be signed by all owners authorizing the release of thecontents to one Owner .
