2 DESCRIPTION The RIGHT to Know HAZARDOUS SUBSTANCE List (RTKHSL) was modified in 2010 to reflect revisions made from sixteen source lists. These sources can be found in the reference sources section located at the end of the RTKHSL. The revised RTKHSL, in alphabetical and CAS number order, and the HAZARDOUS SUBSTANCE Fact Sheets can be obtained online at Questions about HAZARDOUS substances should be directed to the RIGHT To Know Program, Division of Epidemiology, Environmental and Occupational Health, New Jersey Department of Health and Senior Services, Box 368, Trenton, 08625-0368, (609) 984-2202.

3 NOTES: 1. RIGHT to Know SUBSTANCE numbers are not in numerical sequence. This was done to allow for inclusion of generics and the addition of new substances. 2. Substances with prefixes such as 1, 2, 3, o, m, p, n, sec, tert, alpha, beta, and gamma are alphabetized under the first upper case letter of the common name and not by the prefix. PUBLIC EMPLOYERS should direct questions on how to complete the RIGHT to Know Survey and how to label containers to the New Jersey Department of Health and Senior Services at the above address and phone number. B. EXPLANATION OF COLUMN HEADINGS RIGHT to Know SUBSTANCE Number - A four-digit number that is assigned to each HAZARDOUS SUBSTANCE and used for reporting purposes. These numbers are not in sequence. This column also contains the following notations: Syn - Indicates that the name listed is a synonym and you must refer to the second name listed see _____ to report this SUBSTANCE . # - This symbol is found between the SUBSTANCE number and the common name of the SUBSTANCE and indicates that a SUBSTANCE is on the Special Health Hazard SUBSTANCE List.

5 Common name/Chemical name The name on the first line is the common name for the SUBSTANCE ; the name on the second line is the chemical name. When there is only one name, the common and the chemical name are the same. CAS Number - Chemical Abstracts Service identification number. DOT Number - A four-digit identification number used by the Department of Transportation. Special Health Hazard Codes - These codes (SHH codes) are in the RIGHT column and refer to the HAZARDOUS categories of the substances: CA - Carcinogen F4 Flammable - Fourth Degree MU - Mutagen F3 Flammable - Third Degree TE - Teratogen R4 Reactive* - Fourth Degree CO - Corrosive R3 Reactive* - Third Degree R2 Reactive* - Second Degree * Reactive is used interchangeably with the NFPA term instability.

6 According to the hazard criteria defined in 8:59 (a): Carcinogenic, mutagenic and teratogenic substances are considered to be special health hazard substances when they are present as pure substances or in mixtures at a concentration of one-tenth of one percent ( ) or greater. Flammable, reactive/explosive, and corrosive substances are considered to be special health hazard substances when they are present as pure substances or in mixtures at a concentration of one percent (1%) or greater. SECOND Look up the common names of these chemicals on the RIGHT to Know HAZARDOUS Substances List. THIRD Report pure chemicals and products with chemical ingredients that are found on the RTK HAZARDOUS SUBSTANCE List on your annual RTK Survey.
