Selecting Benefits that match your lifestyle, family needs and financial obligations is a very important task. Read this Benefits Guide for information to help you select the Benefits that best meet your needs and the needs of your family. SPECIAL ANNOUNCEMENTS Electronic Enrollment in Kronos will be open from October 23th to November 10th. You can access this by signing into Kronos Employee Self Service and making your selections under each insurance option.

4 NOTE: it is a requirement to view every page (even if waiving coverage). If you do not submit an electronic re- Enrollment your medical plan will default to the Advantage plan and any other current insurance coverage or spending account elections will duplicate for 2018. CMH is having a short plan year in 2018 to align future open enrollments with the fiscal year. All deductibles, out of pockets and spending account maximums will be cut in half to fit into the six month plan year. Re- Enrollment will be held next spring for July 2018.

5 NEW FOR 2018 We are adding two new Consumer Driven Health Plans. This means that you and your covered family members have more responsibility in determining when to receive care, which providers to use, and how to pay for your care. Both plans are High Deductible Healthcare Plans with Account Funding Options: Smart Saver Plan HSA with Limited Purpose FSA Advantage Plan HRA with Traditional FSA CMH Funding for HSA and HRA $1,200 individual / $2,000 family based on participation in the Healthy Decisions Program.

6 Pharmacy Adding a fourth tier of coverage for Specialty Drugs, Adding Step Therapy, Adding Optum Rx Prior Authorization, Adding Optum Rx Quantity Limit program Domestic Partner Coverage no longer available Healthy Decisions New relationship with Occupational Medical Consulting (OMC), Employed Nurse Practitioner, Employed Health Coaches , Participation Funds are contributed to your HSA or HRA accounts on a Quarterly basis. Dental deductibles increasing to $50/$150, major services now covered at 50% and Orthodontia Lifetime maximum increasing to $1,500.

You may choose one of two plans: Smart Saver Plan HSA or the Advantage Plan HRA. Both plans offer the same coverage for medical services and prescription drugs but differ in deductibles, maximum out-of-pocket, co-insurance and premiums.

8 It s important that you carefully review all your choices. Remember, when you enroll in one of the CMH Medical Plan options, you are also automatically enrolled in the Prescription Drug Program. No matter which medical option you select, you are responsible for deductibles, coinsurance, and copays at the time of service. The following chart shows your 2018 cost share.
