Note: Mailed vouchers will be entered into the online system and you may receive email notifications regarding the status of the voucher. 3 Note: This site is restricted to your eyes only as a CARF Surveyor , intern, or trainee. The documents and conversations you find are confidential and privileged. By logging on to the Surveyor Website , you consent to the User s Agreement. CARF Surveyor Website Instructions (Rev 062117) 2 | P a g e Accessing the Surveyor Website To access the Surveyor Website , you must have a Surveyor ID number. If you do not know your Surveyor ID number, please contact your customer service unit. Surveyor trainees are provided with an ID number in the first packet of information mailed before attending Surveyor training. Logging on 1. Go to in your web browser. (Note that www is not included in the web address, and Surveyor is singular.) 2. From the login screen, enter your Surveyor ID number in the following format in the Email field: (Example: Note that, although your login is formatted as an email address, this is not a working email address.)

4 3. In the Password field, if you are logging on for the first time or have not yet set a personal password, enter the default password: 1 Carfsurveyor! If you have set a unique password, enter that password. Then click the Sign In button. (Please be aware that the password is case sensitive.) 4. Surveyor Website homepage is displayed (screenshot below). CARF Surveyor Website Instructions (Rev 062117) 3 | P a g e Securing your account (Only on first login) In order to self-manage your password for this site, you must change your password and authenticate your account the first time you log in. You will only need to complete the following set-up process once: 1. The first time you log in to the Surveyor Website , you will be prompted to change your password (screenshot below). Enter the default password 1 Carfsurveyor! in the Current password field. Then, choose a personal password and enter it into the New password field. Reenter your personal password into the Confirm password field.

Once you have clicked the Update Password and sign in button, you should see the following screen. Enter an email address to authenticate your account and enable password reset. The system will send a verification code to this email address in the future if you forget your password. CARF Surveyor Website Instructions (Rev 062117) 5 | P a g e Click the Set it up now link, enter an email address, and click the email me button. Retrieve the 6-digit verification code sent to the email address you provided, enter it in the code field, and click the verify button (See below). CARF Surveyor Website Instructions (Rev 062117) 6 | P a g e A message confirming that the email address has been verified will appear in the window. Click the finish button (See below). 3. The Surveyor Website homepage should be displayed after completing step 2 above, however, you may see a login screen with the Set it up now button again.

6 If this screen appears, click the Set it up now button and confirm the email address you added by clicking the Looks good button. The set-up process is complete and the Surveyor Website homepage will be displayed. CARF Surveyor Website Instructions (Rev 062117) 7 | P a g e Managing your password You can modify your password at any time by choosing the Change Password link from the Your Info dropdown menu on the homepage. A new browser tab will open where you will need to enter your current password, a new password, and confirm the new password. After you click the Submit button, a profile page will display (see screenshot below). Close this web browser tab to return to the previous tab which is still displaying the Surveyor Website . You cannot change the name, phone numbers, or alternate email listed on your profile. This is your contact information that has been imported into the system from CARF s main database. If you need to change your alternate email or phone numbers listed on the profile page, please use the Update Contact and Employment Info form available on the Surveyor Website under the Your Info menu heading.

7 On the profile page, clicking the tile to Change password will take you through the change password process again. Clicking the tile for Register for password reset will allow you to see what email is being used for authentication in order to perform a password reset. Clicking the Looks good button confirms the email used for password reset and will return you to the profile page. CARF Surveyor Website Instructions (Rev 062117) 8 | P a g e Important Note: The alternate email listed on your profile and the authentication email used for password reset can be the same or different email addresses. You can change the authentication email by clicking the Change link next to the email address, but you cannot change the alternate email listed on your profile. Access Troubleshooting If you forget your password or have trouble logging into your account, you can access the password reset page by using: The Can t access your account?

8 Link on the sign-in page. Choose work or school account when prompted about what kind of account you have. The Forgot your password? link above the Email field on the login page (the link will appear after you have attempted to sign in with an incorrect password). To get back into your account, you will need to confirm your user ID (which should automatically be populated for you), enter the characters in the image, and click the Next button. Click the Email button to have a verification code sent to your alternative email address. The following page should now appear. CARF Surveyor Website Instructions (Rev 062117) 10 | P a g e Retrieve the verification code in your email, enter the code into the field prompt, and click the Next button. A page will appear for you to choose a new password.

9 Enter and confirm a new password and click the Finish button. You will see a confirmation message that your password has been reset. It will include a link to sign in with your new password. Note: CARF cannot track or change personalized passwords created by surveyors. If you cannot access your account after resetting your password, please contact CARF at (888) 281-6531 ext. 7080 or for assistance. Job Postings The Job Postings section of the Surveyor Website has been created as a tool for surveyors to share job openings and solicit applicants. You are invited to post openings that may be relevant to other surveyors. There is also a CARF section where CARF positions are posted. The Job Postings page can be found by clicking on the Job Postings link in the top menu of the homepage. Review existing postings: The Job Postings page displays open positions categorized by accreditation area. Select the individual tabs to show the relevant openings.

10 The CARF tab will show CARF openings, while the All tab will display all open positions in order of post date. From the Job Postings page, click on the blue Submit a Job Posting button. 2. The New Job Posting form should appear. Fill out the relevant information in each category. Categories with a red asterisk are required (Job Title, Company, Description, and Expiration). 3. Once you have clicked on the Description category, editing options will appear at the top of the page. Use the options to format your text or enter HTML code. Hyperlinks can be added using HTML code. 4. In the description, please include relevant contact information such as a name, phone number, email, or web address.
