2 It also clarifies certain items on the general IRCC Checklist . please ensure you read both Document checklists carefully and comply with the instruction on both Document checklists. IMPORTANT REMINDERS: False statements or submission of fraudulent documents will result in refusal, including failure to declare any previous visa refusals, having been denied entry or ordered to leave Canada or any other country. You may also be rendered inadmissible to Canada for a period of five years. please provide photocopies of original documents if needed as we do not guarantee the return of original documents. Failure to submit all required documentation may result in the refusal of your application or processing delays. You must include a translation with any Document that is not in English or French. Kingston Office Document Checklist All applicants must submit the following documents, in addition to the documents listed on IMM 5484: Original valid passport and a photocopy of the biodata (photo) page.

3 Include all cancelled or expired passports, if applicable. The passport should be valid for a minimum of 6 months and be valid for the duration of your stay. Family Information Form [IMM 5707], fully completed, dated and signed. If not applicable, write N/A. If you have answered yes to any of the background information questions on the Application form, provide a completed and signed Schedule 1 form (Application for Temporary resident Visa IMM 5257B) Fee Payment: Online fee payment receipt. No other form of payment will be accepted. Biometric fee payment, if applicable: Biometric fee payment must be paid online and biometric enrollment must be completed at the Visa Application Centre (VAC) Minor (under 18 years of age) travelling with only one parent or alone: provide a signed letter of authorization for international travel from the non-accompanying parent(s) or guardian(s) and a copy of their passport or national identity Document bearing their signature.

4 If you are not currently residing in the country of citizenship of the passport submitted along with your application, provide a photocopy of your current migration status in Jamaica, Cayman, Bahamas or Turks & Caicos 3 please review and submit the category specific documents below: All applicants should submit the following documents: CAN+ Program: If you have travelled to Canada in the last ten (10) ten years or currently hold a valid United States non-immigrant visa and can provide proof (entry stamps in passport, proof of the valid visa) no proof of financial support is required to be submitted. For all others, provide proof of financial support to cover the expenses for the duration of your visit: Any additional relevant documentation (employment letter, pay slips, proof of pension, business registration, investments, etc.)

5 If you are not paying for your own trip: official identification with photo and a bank certificate proving that they have sufficient funds. is submitted. Purpose of travel submit additional documents based on the purpose of your trip (choose at least one purpose): Tourism: A description of your trip, including dates of travel, details of your itinerary and evidence of travel reservations if applicable. Visiting family/relative/friend: A letter of invitation from your host(s) in Canada including; the dates of your visit, their complete names and dates of birth, and complete addresses. Note: If you are applying for a super visa for your parents and grand-parents, please refer to the specific instructions on the Document Checklist (IMM5484) and our website and submit the documentation listed. Business/Attending an event: A letter of invitation from the inviting company or organization and a letter from your employer outlining the purpose & details of your trip.

6 Note: If travel expenses are to be paid by the inviting party or the employer, this must be specified in the letters accompanying the application. 4 Short-term studies: A copy of the Letter of Acceptance from the school in Canada , which must include the program of studies, the start and end dates of studies, the tuition fees, and related costs for the duration of your studies. Transit (stop-over of less than 48 hours in Canada ): A copy of the flight itinerary and the visa for the destination country, if applicable. Returning Student: proof that you are enrolled in a Designated Learning Institution in Canada (transcript, official letter, certificate, etc.) and a copy of your valid study permit in Canada . Returning Worker: A letter from your employer in Canada confirming your ongoing employment and a copy of your valid work permit in Canada .

7 Entertainers: Purpose of Trip: Details of your itinerary in Canada & evidence of travel arrangements Employment offer: this letter must be signed by the person requesting you to perform in Canada and it must be on letter head. Give details of the performances, location and tour stops if applicable. Signed Copy of your performance contract or venue rental agreement (if applicable) Original Police Certificate: You must provide a police certificate from your country of residence in the previous 12 months. If you or any accompanying family members have a criminal conviction(s), you must provide an original police certificate from the country where you or your family member was convicted and provide a personal explanation of events leading to the conviction(s) and any related documentation such as court records, police report etc.

8 Evidence that you are an established entertainer, such as: copies of previous recordings, performance contracts, marketing and promotional material, a letter of introduction from your record label or management company. Ensure you provide your stage name if different from the name on your application.
