IMPORTANT REMINDERS: False statements or submission of fraudulent documents will result in refusal, including failure to declare any previous visa refusals, having been denied entry or ordered to leave Canada or any other country. You may also be rendered inadmissible to Canada for a period of five years. please provide photocopies of original documents if needed as we do not guarantee the return of original documents. Failure to submit all required documentation may result in the refusal of your application or processing delays. You must include a translation with any Document that is not in English or French. Kingston Office Document Checklist All applicants must submit the following documents, in addition to the documents listed on IMM 5484: Original valid passport and a photocopy of the biodata (photo) page.

6 Note: If travel expenses are to be paid by the inviting party or the employer, this must be specified in the letters accompanying the application. 4 Short-term studies: A copy of the Letter of Acceptance from the school in Canada , which must include the program of studies, the start and end dates of studies, the tuition fees, and related costs for the duration of your studies. Transit (stop-over of less than 48 hours in Canada ): A copy of the flight itinerary and the visa for the destination country, if applicable. Returning Student: proof that you are enrolled in a Designated Learning Institution in Canada (transcript, official letter, certificate, etc.) and a copy of your valid study permit in Canada . Returning Worker: A letter from your employer in Canada confirming your ongoing employment and a copy of your valid work permit in Canada .

7 Entertainers: Purpose of Trip: Details of your itinerary in Canada & evidence of travel arrangements Employment offer: this letter must be signed by the person requesting you to perform in Canada and it must be on letter head. Give details of the performances, location and tour stops if applicable. Signed Copy of your performance contract or venue rental agreement (if applicable) Original Police Certificate: You must provide a police certificate from your country of residence in the previous 12 months. If you or any accompanying family members have a criminal conviction(s), you must provide an original police certificate from the country where you or your family member was convicted and provide a personal explanation of events leading to the conviction(s) and any related documentation such as court records, police report etc.

8 Evidence that you are an established entertainer, such as: copies of previous recordings, performance contracts, marketing and promotional material, a letter of introduction from your record label or management company. Ensure you provide your stage name if different from the name on your application.
