1 Important Information about your uwhc Regular Benefits As an employee of uwhc , Benefits are an Important part of your total rewards package. Below are links to additional details about the Benefits that you re eligible for. You can enroll dependents such as a spouse, child(ren), step child(ren), and legal wards onto your appropriate benefit plans. You are not required to print these additional details. However, please remember to print out and complete the packet of enrollment forms provided in your Benefits notification and bring the enrollment packet with you to New Employee Orientation.
2 As a friendly reminder, you have 30 days from your start date to submit your enrollment forms to Human Resources. Submitting Applications You will have the opportunity to return your completed benefit applications during your New Employee Benefits Orientation. Other than the three required forms, you only need to submit applications for those Benefits that you are enrolling in. 3 If you elect to enroll in a supplemental retirement plan, those deductions are taken on every biweekly paycheck. After submitting your initial Direct Deposit Form during orientation, you can make updates to your Direct Deposit at any time by submitting a new Direct Deposit Form directly to the Payroll department. You can make updates to your W-4 by submitting a new W-4 form to the Payroll department at any time. Payroll Schedule Benefit Details BENEFIT SUMMARY uwhc offers a variety of benefit programs including sick pay, vacation pay, legal holiday pay, assistance programs, and more.

Regular Benefit Summary uwhc Paid Time Off UNIFORM HEALTH & DENTAL INSURANCE uwhc offers four different health insurance plans to eligible employees. More Information regarding the specifics of each plan can be found in the Regular Employee Benefit Summary or in the below links. To enroll in health insurance through uwhc , a completed Health Insurance Application/Change Form must be submitted to Human Resources within 30 days of your start date. Uniform dental insurance is available through uniform health insurance coverage.
5 Employees have the opportunity to elect health coverage with or without dental. If enrolling in a family health plan, all eligible members of a family must be enrolled on the plan. If you elect to enroll in early coverage with no uwhc contribution, you are responsible for paying the full premium amount until uwhc contribution begins. If you waive health insurance coverage at time of new hire, you may be eligible for an Opt-Out Incentive up to $2,000. If you need to enroll in coverage mid-year, the only opportunities to enroll would be when experiencing a qualifying event (marriage, divorce, birth, adoption, etc.)

Information regarding each of the plans can be found below.
7 In addition, included in this Information is a comparison grid of the different dental plans available to uwhc employees. Enrollment in the Supplemental Delta Dental plan is limited to new hires or mid-year enrollments when experiencing a qualifying life event (marriage, divorce, birth, adoption, etc.). Enrollment in the EPIC Benefits + plan is limited to time of new hire or mid-year enrollments when experiencing a qualifying life event. This benefit is traditionally not offered during Open Enrollment. It is Important to understand how supplemental Benefits work together to help you determine what coverage is best for you.
8 Review the Dental Comparison Table for more Information on the premiums and coverage for each of the plans available to you. The applications for Supplemental Delta Dental and EPIC Benefits + are not required if you are waiving coverage, but should be submitted within 30 days of your start date if you plan on enrolling in the applicable Benefits . 9 If eligible, you can enroll in supplemental vision insurance plans administered through Vision Service Plan (VSP) and/or Epic Benefits +. Information regarding the specifics of supplemental plans can be found below, including premium rates for coverage. In addition, included in this Information is a comparison grid of the different vision plans available to uwhc employees. If coverage is waived at time of new hire, employees may be eligible to enroll in VSP or Epic Benefits + mid-year if they experience a qualifying life event. Otherwise, employees can enroll in VSP during Open Enrollment period.
