With Consignment , we are leveraging Walgreens current distribution infrastructure in a direct product acquisition model delivering to retail Pharmacy and ultimately patients Inventory has no cost. Through the use of manufacturer savings cards, eligible patients are able to save significantly on select Bausch (Valeant) to locate information on Storenet: StoreNet > Rx > Patient Care > Pharma Partner/ Consignment Programs > Bausch/ValeantThe Valeant/Bausch Access Program is comprised of two categories of products offered from Ortho-Dermatologicsand Bausch & Lomb that have a lower patient cost at Walgreens. 7 Bausch (Valeant) 2020 Walgreen Co. All rights Patients are eligible to have an eVoucherand Bausch (Valeant)/Access Program savings card applied in order to maximize savings. There is no need to have the eVoucherreversed.

5 Simply add the savings card as the secondary plan and bill it as an auto COB to realize the maximum savings for the patient. Most uninsured patients under the age of 65 are eligible for the manufacturer discount program. Patient eligibility will be determined by Bausch (Valeant) during discount coupon activation. Prescriptions for government funded patients (Medicare Part B, Medicare Part C (Medicare Advantage), Medicare Part D, Medicaid Fee for Service, Managed Medicaid, TRICARE, Indian Health Service) are not eligiblefor this Bausch (Valeant) discount coupon program. MFR Savings Card (Valeant ) program -This service utilizes Walgreens Centralized Services outreach to help the patient save on out-of-pocket VAP/BAP expenses, and reduce work for team members.

6 By increasing the VAP/BAP savings card offers, patients receive greater savings and improved adherence. All patients receive a patient outreach call, however, some patients are excluded from a savings card offer. Due to certain laws in these states, Massachusetts and Minnesota are excluded from the Valeant/Bausch Access Program. All rights Pay Prescription Program Walgreens patients will have access to select dermatology products from Ortho Dermatologics cash-pay prescription program, with prices ranging from $50-$115 per prescription.

7 Process the prescription to the patient s insurance. If the prescription rejects ( Drug Not Covered or Prior Authorization Required ), simply cash out the prescription for the new lower cash Centers of Excellence (COE) While all Walgreens pharmacies have access and can continue to dispense consigned Bausch products, additional training review has been provided to Bausch COE network of pharmacies. These select COE locations will continue to assist patients with their consigned Bausch prescriptions and provide patient savings program opportunities for eligible patients. Ensure these COE locations have select Bausch products on hand at all Consigned Bausch products are available to order through WAG DC (PDQ)9 Kaleo Product: Auvi-Q 2020 Walgreen Co.

The majority of patients will have a $0 dollar out of pocket for Auvi-Q when covered by the insurance but with select rejects pay up to $50 dollars Team members are able to check patient eligibility and generate the Manufacturer Direct Bill Card for the Coordination of Benefits (COB) claim Where to locate information on Storenet: StoreNet > Rx > Patient Care > Pharma Partner/ Consignment Programs > Auvi-Q (Kaleo) Walgreens has partnered with Kaleo, manufacturer of Auvi-Q, to increase patient access to an epinephrine auto-injector(EAI) via the Kaleo Auvi-Q Manufacturer Direct Bill Program. 10 Kaleo 2020 Walgreen Co. 9 Patients with a high deductible may receive a higher co-pay due to coverage limitations. Government funded patients are not eligible for the program, however select Medicaid patients may be able to receive assistance from the manufacturer through the Kaleo Cares program. For additional information, patients can call Kaleo Cares at (502) 213-7601. Uninsured patients are not eligible for the program, but may be able to receive assistance from the manufacturer through the Kaleo Direct Delivery Service. For additional information, patients can call Kaleo Direct Delivery Service at (844) Centers of Excellence (COE) While all Walgreens pharmacies have access and can continue to dispense Auvi-Q, additional training review has been provided to Auvi-Q COE network of pharmacies.

10 These COE locations will continue to assist patients with their Auvi-Q prescriptions and provide patient savings program opportunities for eligible patients. We are also asking that these COE locations ensure they have the Auvi-Q on hand at all times. The direct bill program will generate coupons with the same Plan ID but different groups depending if the patient is commercially insured , uninsured, or covered by Medicare Part D.
