E-mail: NOTICE: Any communication or discussion concerning this RFP with any Commonwealth of Kentucky personnel other than Kelly Hawkins, CPPB may result in the firm s response to this REQUEST for proposal to be determined to be non-responsive. 3 PROJECT DESCRIPTION This project will construct a new Maintenance Building in Marion, Crittenden County, Kentucky on property owned by the Commonwealth at 466 Pippi Hardin Boulevard. Currently, there are no funds available for the construction of this project.

4 The Kentucky Transportation Cabinet are expecting the funds for construction to be available in the FY19 budget year. Funding is currently available for the design of the project only. A site survey has been completed for the project and is available as an attachment to this REQUEST for PROPOSALS . No subsurface investigation has been completed for this project. It is expected that this subsurface investigation will be accomplished during the early phases of the project design once the Design Team has determined the appropriate siting of the building on the property. The land purchase has already been accomplished. The maintenance building will contain approximately 8,000 gross square feet and will house the Crittenden County Highways maintenance crew. The facility will replace the existing maintenance garage that is over 35 years old, in poor condition, and not meeting the functional needs of the maintenance crew.

5 Built in equipment will be a part of this construction. Other equipment and tools will be either relocated from the existing facility or purchased by the Commonwealth outside of this project. The Design Team will be provided with site boundary and topographical information and existing underground and overhead utilities from Advanced Engineering and Land Surveying, Inc working under separate contract with the Owner. As indicated above, this information is already available, has been attached to this REQUEST for PROPOSALS as an exhibit, and will be provided in electronic format to the successful team. The Design Team will assist the Owner in obtaining geotechnical information through a Geotechnical Engineering Firm working under separate contract with the Owner. The Kentucky Transportation Cabinet with the assistance of the Finance and Administration Cabinet (Division of Engineering and Contract Administration) has accomplished similar buildings in Muhlenberg County, Henderson County, Menifee County, and Harrison County that are all currently under construction.

6 These buildings will serve as the prototypes for this building. While these prototypes each have a similar layout of spaces required in the building, it is anticipated that the Architect will design the building using programming information from the Transportation Cabinet specifically for this project. Potential respondents that wish to visit one of these prototype facilities. Since these projects are currently in various stages of construction, anyone wishing to visit one of these facilities or to obtain the completed floor plans of one of these buildings should make arrangements with the DECA Project Manager (Dwayne Brown for Muhlenberg or Henderson Counties and Joseph Sandman for Medifee and Harrison Counties). These DECA Project Managers can make the arrangements with the various construction sites, but will not accompany anyone on the site visit.

7 If any potential respondent wishes to investigate these facilities under construction they must be aware that NO INFORMATION provided at the facility should in any way be construed as pertinent to this specific RFP. As indicated above, any potential respondent make one of these site investigative visits SHALL NOT COMMUNICATE with any Commonwealth of Kentucky personnel on site or at the County Transportation District Office/ Maintenance Facility. 4 This project consists of the design of the project ONLY at this time. However, should funding be obtained for FY19 (July 2018-2019) the project will consist of construction administration by the successful firm. LEED Certification is NOT required to comply with the HPBS (Kentucky High Performance Building Standards). No submittal will be made to USGBC for certification. However, the Design Team will be required to prepare a LEED scoresheet to guide the design and construction to make this facility as energy efficient and environmentally friendly as possible within the constraints and budget of the facility.

8 Included with this RFP are the following: Property boundary survey Topographical and Utility survey CONSULTANT TEAM COMPOSITION The consultant team shall include, at a minimum, the following professional design Disciplines/Expertise. Disciplines/Expertise may be reside in one or multiple persons/firms. The consultant team may include other expertise in additional to that listed below. Architect Mechanical Engineer Electrical Engineer Structural Engineer Site Design professional Special Inspections (must be an independent firm from the design firms) INDICATION OF KENTUCKY REGISTRATION This project requires the services of an Architect and/or Engineer registered in Kentucky. Respondents are to provide, in their response submittal, Kentucky Architectural Registration or Kentucky Board of Registration for professional Engineers numbers for each Architect and/or Engineer that will be providing services for this project.

9 RESPONDENTS ARE TO PROVIDE REFERENCES Respondents are to provide a minimum of three (3) references from similar projects. These references are to include: name of project, address of project, date project completed, Construction amount, current contact person and phone number. 5 PROJECT SCHEDULE Schedule illustrates consecutive calendar days for project tasks/phases, commencing upon execution of consultant's contract: Phase A Schematic Design 90 days Phase A Review 14 days Phase B Submittal 75 days Phase B Review 14 days C Contract Documents 90 days Phase B/C Review by Owner.

10 Final Revisions 30 days Bid/Award Contract 45 days Construction TBD SPECIAL NOTES Respondent s attention is called to the Requirement of Affidavits included in this RFP, must be submitted with each proposal . Failure to submit could result in your PROPOSALS not being scored. A Federal Standard Form 330 Part II is not required to be on file but must be included in your proposal . The minimum submittal requirement is for a 330 Part II for the lead firm of the consultant team. A 330 Part II for each consultant team member is optional, but is often helpful to the Selection Committee in their review. The form is located at the following link: REGISTRATION WITH SECRETARY OF STATE: Domestic and foreign corporations shall be registered with the Kentucky Secretary of State and declared to be in good standing prior to award of contract.
