7 BANK DETAILS: The diesel refund will only be paid into a specified bank account via electronic Bank Transfer. No cheques will be issued. The bank particulars provided on the VAT101D form will replace the existing bank particulars on the VAT system, if such differs. If the bank particulars changed, one of the following must accompany this form: Latest bank statement, original cancelled cheque or confirmation by your bank that you are the account holder. 1. BTW-REGISTRASIENOMMER: Geregistreerde ondernemers moet hulle BTW-registrasienommer invul. In die geval van Boerdery of Mynbou moet die sub-klassifikasiekode, uit die tabel hierbo, ingevul FINANSI LE INLIGTING: Omset vir die huidige en twee voorafgaande fINANSI LE jare volgens Dieselterugbetalingkonsessietipe. 6. DIESELAANKOPE Liters vir die huidige en twee voorafgaande FINANSI LE jare volgens Diesel Terugbetalingkonsessie tipe. 7. BESONDERHEDE VAN DIESEL VERSKAFFER Naam van dieselverskaffer en kli ntenommer. 8. ANDER INLIGTING: Die velde moet voltooi word indien die applikant betrokke is by ander aktiwiteite teen vergoeding (vervoer, oes-/stroopwerk, konstruksie, ens)9.

9 BANKBESONDERHEDE: Die dieselterugbetaling sal slegs in 'n gespesifiseerde bankrekeningnommer via Elektroniese Bankoordrag betaal word. Geen tjeks sal uitgereik word nie. Die bankbesonderhede wat op die BTW101D vorm verskaf word, sal die bestaande bankbesonderhede op die BTW-stelsel oorskryf, indien dit nie ooreenstem nie. Indien die bankbesonderhede verander het moet een van die volgende hierdie vorm vergesel: Nuutste oorspronklike bankstaat, oorspronklike gekanselleerde tjek of bevestiging van u bank dat u die rekeninghouer is.
