2 2. In advance of a rewrite of AFI 91-202_AFMCSUP, the following paragraph changes and additions in the attachment are effective immediately. 3. This memorandum becomes void after one-year has elapsed from the date of this memorandum, or upon release of an AFMC publication incorporating the guidance, whichever is earlier. TODD M. VENEMA, Colonel, USAF Director of Safety Attachment CHANGES (Change) (Added-AFMC) High risk acceptance documentation for High Mishap risks shall be coordinated with, Center/SES, AFMC/SES, AFSEC/SEF, Center/EN-EZ, and the Lead command s A3, A4, A5, SE prior to being sent to AF/SE. AFMC/SES shall inform AFMC/CC/CV, EN, A3/6, A4, and A5/8/9 of impending new High risk package. After obtaining AF/SE coordination the Program Office will send the risk acceptance package to the PEO for the final coordination before sending to the CAE for risk acceptance.

3 The CAE shall sign an attached memo in the risk acceptance package to show acceptance of the Mishap risk. The Program office will then notify the principle stakeholders after the risk has been accepted by the CAE. (Delete last sentence) (Added-AFMC) Changes in risk acceptance authority. When a new risk acceptance authority assumes their assignment, that person is immediately responsible for all safety risks in their portfolio and must be notified of safety risks that were accepted by their predecessors. Center/Wing equivalent safety offices shall ensure the new risk acceptance authority are notified of safety risks that were accepted by their predecessors within 60 days of assumption of duties. Risks previously coordinated and accepted need not be fully re-coordinated when hazards and conditions have not changed.

4 (Change) (Added-AFMC) Every four years, the PM will review the accepted High and Serious Mishap risks with the applicable risk acceptance authority. If the risk level is changed, full coordination/acceptance shall be accomplished, otherwise it can be accepted by proper risk authority and information only notification to all that were involved in the previous coordination. If the risk has been downgraded, the offices that coordinated on the higher risk shall be notified that the risk has been mitigated. (Change) (Added-AFMC) SSE/SSMs shall work with the test community and AFOTEC to identify those tests that will verify the residual risks of specific hazards. SSE/SSMs shall provide access to the Program hazard analyses for the test community and AFOTEC. SSE/SSM shall provide test support IAW AFI 90-802, paragraph (Add) (Added-AFMC) Safety analysis of any technical order work-arounds or other hazardous logistical safety issues.

5 (Add) (Added-AFMC) For aircraft SSGs within AFLCMC, AFMC/SES has delegated primary MAJCOM participation to AFLCMC/SES. AFMC/SES will be involved in all aircraft SSGs that have High risks, are OSD or HAF high-interest weapon systems, or are having their first SSG. (Add) (Added-AFMC) SSG Charters shall be reviewed annually for potential changes. (Change) (Added-AFMC) Upon delivery of the SSEA request, the SSEA team chief will be AFLCMC System Safety or they will identify the team chief. The SSEA Team Chief will assemble a team consisting of user representatives and other experts, as required, based upon the nature of the proposed operations. The user representatives will prepare detailed, step-by-step procedures and the team chief, using system safety management and analytical techniques, will perform and document the hazard analysis and results of the SSEA.

7 Requirements in this publication are mandatory, unless indicated otherwise. It applies to all Regular Air Force (RegAF), Air Force Reserve command (AFRC) and Air National Guard (ANG) military and civilian personnel. For the purposes of this instruction, ANG and Air Force Reserve command (AFRC) are included in all references to Major Commands (MAJCOMs). At enduring and contingency locations outside the United States, follow the requirements in this instruction so long as they do not conflict with applicable requirements from any of the following: host nation requirements made applicable by international agreement, Overseas Environmental Baseline Guidance Document (OEBGD) 2 AFI91-202_AFMCSUP 17 MAY 2017 standards, country-specific Final Governing Standards (FGS), Geographic Combatant command policy, environmental annex to operational order (OPORD), operational plan (OPLAN) or other operational directive.

8 This instruction implements North Atlantic Treaty Organization (NATO) Standardization Agreements (STANAGs) 3101, Exchange of Safety Information Concerning Aircraft and Missiles, 3102, Flight Safety Cooperation in Common Ground/Air Space, 3531, Safety Investigation and Reporting of Accident/Incidents Involving Military Aircraft and/or Missiles. Send major command , field operating agency and direct reporting unit (MAJCOM/FOA/DRU) supplements to HQ Air Force Safety Center (HQ AFSEC)/SE Org Box, 9700 G Avenue, Kirtland AFB NM 87117-5670, for coordination and approval before publication. Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the AF Form 847, Recommendation for Change of Publication; route AF Form 847s from the field through the appropriate functional s chain of command .

9 All requests for changes, interpretations or clarifications concerning this publication must be forwarded through the MAJCOM/FOA/DRU safety organization, who, in turn, as applicable, will forward to HQ AFSEC. The authorities to waive wing/unit level requirements in this publication are identified with a Tier (T-0, T-1, T-2, T-3) number following the compliance statement. See AFI 33-360, Publications and Forms Management, for a description of the authorities associated with the Tier numbers. Submit requests for waivers through the chain of command to the appropriate Tier waiver approval authority, or alternately, to the Publication OPR for non-tiered compliance items. Refer to AFPAM 90-803, Risk Management (RM) Guidelines and Tools, for guidance on using the RM process.
